The dramatic increase in conditions including non-communicable diseases such as cancers, cardiovascular disease and diabetes, communicable diseases such as HIV/AIDS and tuberculosis, and long-term mental health disorders, is one of the key challenges facing health care management systems throughout the world. However, most do not have a plan for managing these chronic conditions and simply treat symptoms when they occur. This report presents the results of the "Innovative Care for Chronic Conditions" project launched by the World Health Organization in order to analyse best practice and develop a framework for the prevention and management of chronic conditions. It seeks to alert governments throughout the world about these important changes in global health, and presents health care solutions for managing this rising burden. |
